Registering With The Practice


New Patients


The Practice is currently welcoming new patients. Patients are eligible to register with the Practice if they reside within our Practice boundary (the blue line). An outline of this can be found below:

Any patient within this area can register by calling into either Practice and completing one of our registration packs. The pack contains a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.  Once you have completed your forms, please return these to the Practice is person along with any supporting documentation such as:

  • A piece of photographic identification (e.g passport, driving licence)
  • A recent official document with your current home address to show that you are residing within our Practice boundary

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

If in the future you move outside the boundary above then you may be asked to re-register with another GP closer to your new address.

Changing Your Address


Any current patient who changes their address is encouraged to update this with the Practice as soon as possible, to ensure that any important communications are able to reach you. A change of address form is available from either of our Reception desks. Existing patients are eligible to remain with the Practice so long as your new address resides within either the red or blue outer boundaries.

The Practice does offer the option for patients who move outside the boundary lines to remain with the Practice under the Extended Patient Choice scheme. To apply to remain, patients need to complete a change of address form at either Reception desk and indicate their wish to remain. All patients will then be considered on a case-by-case basis in order to determine if it is appropriate for them to remain registered under this scheme.

If accepted, any patient requiring a home visit will not be visited by one of our GPs. Instead they will be informed to contact NHS 111 who will arrange for someone to visit instead.

If you circumstances once accepted, the GP partners retain the right to reassess any case at any time. If they feel it is no longer in the best interests of your case to remain registered with us, then you may again be asked to register with a service closer to your home address.
