Information for Patients

Named GP

As of April 2015, all GP Practices were encouraged to assign every patient to a named GP. It is this GP's responsibility to ensure that all these patients received appropriate care and support from the Practice.

All patients at the Practice have been assigned to Dr Hakes as their named GP.

Whilst Dr Hakes is responsible for the overall care and support each patient receives, on a day-to-day basis this does not affect the way you access any of our services. Appointments are available with any GP of your choice.

Training Practice

The Practice is registered to provide training to doctors, nurses and other health care professionals and participates in training of medical students in association with the University of Sheffield. When training is taking place you will be informed by a member of the Reception team, or suitable notices in the waiting area, the nature of the training taking place and the people involved.

If you do not wish for trainees to be present during your consultation, please inform a member of the Reception team and they will be able to accommodate this for you.

NHS Friends & Family Test

The NHS Friends and Family Test was introduced to GP Practices in December 2014. This test gives all patient the opportunity to rate the care, treatment and services provided by their GP Practice. Patients are asked how likely they would be to recommend the service to their friends and family.

Patient feedback is important to practices as it lets them know what their patients feel about the service they receive, as well as inform their decisions when making changes to improve the care they provide.

Following any of out appointments you may receive a text asking if you would like to complete the Friends and Family feedback. Alternatively you can find forms located at both reception desks. This is not required, but all feedback is welcome.

If you would like to complete the NHS Friends and Family test online, you can do it now here.

The Hillsborough GP Network

Dr Hakes & Partners are very pleased to announce that the Practice is now part of the Hillsborough Network; a collaboration between the Hillsborough Practices alongside Dykes Hall Medical Centre, Far Lane Medical Centre and Dr Bradley and Partners (Tramways Medical Centre).

This will enable us to offer new services to the Hillsborough community, such as physiotherapy or social support workers. We hope to develop new services over the coming years and hopefully these will have a positive impact on our patients and the local community.
